Thank you for joining DANZART ACADEMY we hope you will have a wonderful dance experience with us during our 4th year of operation.
Below you will find a list of our studio policies, please adhere to all rules and regulations so that we can provide a safe and enjoyable learning environment.

REGISTRATION INFORMATION:

Students are required to pay an annual registration fee.
*Registration fees are not refundable.

PAYMENT PROCEDURE:

Tuition is due by the 1st of every month a $10.00dlls late fee will be assessed if payment is not received by the 5th of each month.
Tuition must be paid automatically by debit or credit card.

MISSED CLASSES:

By signing this form; you are committed to attend classes, and to pay in a timely manner.
If you attend only 1 time to a class the whole month will be charged, we do not charge by class, we charge by month once you enroll.
It is your responsibility to call ahead, if for any reason the student wouldn’t be able to attend classes for a period of time, grater than 30 days. Due to health issues or other.
By reporting to our office, you will be avoiding late charges and monthly tuition.

DANCE YEAR:

DANZART ACADEMY year commences September 2010 and ends June 2011.

UNIFORM:

Dancers are required to wear DANZART uniform (obligatory) dance shoes are required inside the classroom. Only black sweatshirts NO SIGNS. Hair must be pulled off the face. No jewelry, no gum, no toys, and no street shoes are allowed!

DROP OFF:

Please park in the designated marked stalls of the car park. DANZART ACADEMY can not be responsible for unattended students. Danzart academy insurance prohibits staff from walking students to parked cars.
You must escort your child from your car to the academy and vice-versa.
Please arrive 10 minutes before class start. Parents are required to arrive back 5 minutes before the class ends! (PLEASE ASK YOUR CHILD TO WAIT INSIDE THE ACADEMY FOR SAFETY) NO PARENTS ALLOWED IN CLASSROOM.

HOLIDAY DATES:

Please refer to the notice board regarding Academy closure dates, for holiday and for important events. Or general information.

FOOD:

Absolutely no food allowed in the classroom, only water will be permitted into classroom.

END OF YEAR RECITAL:

By the end of June there will be a recital in which students can participate. (With out obligation). Costumes and tickets will need to be purchased for this event.

OFFICE HOURS:

Office hour are Monday-Thursday from: 9:00 am-11:00 am 4:00 pm-8:00 pm or by appointment.

FITNESS STUDENTS

: Please sign in before you enter class, if you want a change in your schedule class, or want to quit your classes, please notify the office so we can cancel your automatic payment.

LIABILITY:

A release and waiver of liability form, must be signed by a parent/legal guardian before student is allowed to participate in class. Danzart Academy does not carry medical insurance for students.



By signing this agreement you have read and understood the entire agreement and are willing to abide by DANZART ACADEMY POLICIES.
